The exact mechanism of action of Mesalamine is unknown, but it appears to work topically rather than systemically. In patients with chronic inflammatory bowel disease, the mucosal production of arachidonic acid metabolites is increased through cyclooxygenase (prostanoids) and lipoxygenase (leukotrienes and hydroxy eicosatetraenoic acids) pathways. Mesalamine reduces inflammation by blocking cyclooxygenase and inhibiting prostaglandin production in the colon.

Pentasa is used for:
- Treating mild to moderately active Ulcerative Colitis and Crohn’s Disease
- Maintaining remission in Ulcerative Colitis
- Maintaining remission in Crohn’s Disease
Using Mesalamine alongside nephrotoxic agents like NSAIDs and Azathioprine may increase the risk of kidney problems. Mesalamine should not be used by individuals with hypersensitivity to salicylates or any component of the formulation.

Tablet Dose:
Treatment of mild to moderate Ulcerative Colitis
Adults: 800 mg (two 400 mg tablets) three times daily for 6 weeks. The dose may be increased up to 4.8 g/day in divided doses if necessary.
Children (5 years and older):
- Weight 17 to <33 kg: Morning - two 400 mg tablets; Evening - one 400 mg tablet (6 weeks)
- Weight 33 to <54 kg: Morning - three 400 mg tablets; Evening - two 400 mg tablets
Maintenance of remission in mild to moderate Ulcerative Colitis
- Adults: 1.6 g (four 400 mg tablets) daily in two to four divided doses. Dose may be increased to 2.4 g/day if needed.
Maintenance of remission in Crohn’s Disease
- Adults: 1.2–2.4 g daily in divided doses.
- Children: 400–800 mg three times daily.
Prolonged Release Granules Dose:
Treatment of Ulcerative Colitis:
Adults:
- Active disease: Up to 4 g once daily or divided into 2–4 doses.
- Maintenance: 2 g once daily.
Pediatric (6 years and older):
- Active disease: 30–50 mg/kg/day in divided doses, up to 75 mg/kg/day. Maximum dose: 4 g/day.
- Maintenance: 15–30 mg/kg/day, not exceeding 2 g/day.
Note: The granules must not be chewed. The contents can be emptied onto the tongue and swallowed with water or orange juice, or mixed with yogurt and consumed immediately.

✔️ Overdose Effects
- There is no specific antidote for Mesalamine overdose. Treatment is supportive and may include:
- Preventing further gastrointestinal absorption
- Correcting fluid and electrolyte imbalances
- Maintaining renal function
- As this is a pH-dependent delayed-release product, this factor should be considered when treating an overdose.
✔️ Geriatric patients (65 years and older):
Blood cell counts should be monitored closely. Since renal function may decrease with age, it is recommended to evaluate renal function before starting Mesalamine and monitor it throughout therapy.
✔️ Pediatric:
Safety and effectiveness in children under 6 years have not been established.
✔️ Liver impairment:
There have been reports of liver failure in patients with pre-existing liver conditions. Caution is advised when administering Mesalamine to such patients.
✔️ Pregnancy & Lactation
Mesalamine should only be used during pregnancy if the potential benefit justifies the potential risk to the fetus. Caution is advised for nursing mothers.
✔️ Pyloric stenosis:
Patients may experience delayed gastric emptying, which can delay Mesalamine release in the colon.
✔️ Renal impairment:
Cases of nephropathy and interstitial nephritis have been reported. Renal function should be checked before starting treatment and monitored periodically.
